Explanation:

1. x – mandatory to submit.
2. (x) – not mandatory if the project list is included in the CV.
3. The Code of Ethics is signed upon receiving the certificate (it does not need to be submitted separately by the applicant). Before certification, the applicant must read the Code of Ethics.
